R96-166 RESOLUTION NO. ,,¢¢'~/-/~ A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF BOYNTON BEACH, FLORIDA, AUTHORIZING AND DIRECTING THE MAYOR AND CITY CLERK TO EXECUTE AN AGREEMENT BETWEEN THE CITY OF BOYNTON BEACH AND THE STATE OF FLORIDA, DEPARTMENT OF HEALTH AND REHABILITATIVE SERVICES, PALM BEACH COUNTY PUBLIC HEALTH UNIT (PBCPHU), FOR THE PURPOSE OF PROVIDING NECESSARY IMMUNIZATIONS TO ALL CHILDREN THROUGHOUT PALM BEACH COUNTY; AND PROVIDING AN EFFECTIVE DATE. WHEREAS, the Boynton Beach Fire Department has made a commitment to expand their community health services to the public, and is currently one of three departments locally, who are a part of a trial immunization program this is currently under development; and WHEREAS, this program has been endorsed by the County Fire Chief's Association, the Medical Director's Association, the Florida Department of Health and Rehabilitative Services, the Local IAFF 1891 union, and the Palm Beach County Public Health Unit, and is the beginning of an expansion of community health services being provided by fire rescue services; and NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF BOYNTON BEACH, FLORIDA THAT: Section 1. The City Commission hereby authorizes and directs the Mayor and City Clerk to execute an Agreement between the City of Boynton Beach and HRS Palm Beach County Public Health Unit, which Agreement is attached hereto as Exhibit "A". ~ This Resolution shall take effect immediately upon passage. ATTEST: Cit~ Clerk CITY OF BOYNTON BEACH, FLORIDA Commissioner Cxod~'missioner MEMORANDUM OF AGREEMENT THIS MEMORANDUM OF AGREEMENT, (hereinafter referred to as "Agreement"), is entered into between the State of Florida, Department of Health and Rehabilitative Services, Palm Beach County Public Health Unit, (hereinafter referred to as "PBCPHU"), and the (hereinafter referred to as "Fire Rescue Unit"), on this . day of ,1996, for the purpose of providing necessary immunizations to all children throughout Palm Beach County. A. ResponsibiLities of the PBCPHU: 1. To provide Public Health Nurses who will initiate, organize and schedule immunization programs. 2. The Public Health Nurses will manage activities related to client flow. 3. The Public Health Nurses will determine what immunizations are needed for each client by reviewing each client's immunization records and assessing each client as to health status, past health problems, allergies and reactions to previous immunizations. 4. The Public Health Nurses will inform the parent, guardian or responsible adult which immunizations are needed and advise as to possible side effects and fever control. 5. The Public Health Nurses will answer all questions of the parent, guardian or responsible adult. 6. The Public Health Nurses will complete forms for day care and schools. 7. The Public Health Nurses will give appointments for TB skin test readings if indicated. 8. The Public Health Nurses will provide dates when additional immunizations are required. 9. The Public Health Nurses will complete and sign "Immunization Assessment Forms" for each client indicating which vaccines are to be administered and whether vaccines are given by the Volunteer Nurse or Paramedic. 10. Both Public Health Nurses and Volunteer Nurses may administer immunization vaccines in the immunization progranis. ~ This Resolution shall take effect immediately upon passage. To assign paramedics for the providing of immunization services at the PBCPHU immunization programs, r Each assigned paramedic must have successfully completed and received a certificate of completion for the "Paramedic Continuing Education Course for Immunizations" from Palm Beach Community College. 2. The Paramedic ~ follow the "Immunization Assessment Form" for each client signed by the Public Health Nurse. 3. The Paramedic ~ administer to each client the correct vaccine(s) utilizing the correct rOute, site and dose according to established protocols. 4. The Paramedic will document immunization actions on the immunization records, according to established protocols. 5. The Paramedic will adhere to the "Courtesy Standards" of the PBCPHU. 6. The Paramedic will assist with other activities as to enable the immunization services to be provided in a timely manner. This may include, but not be limited to: a. Assisting with setting up and taking down equipment b. Assisting with duties related to client flow 7. To contact the PBCPHU regarding a paramedic's inability to attend a scheduled immunization program by contacting the following entities: a. For assignments with the Community Immunization Services (CIS), call the Immunization Supervisor or Team Leader at (561) 840-4566. b. For assignments atthe PBCPHU Health Centers, call the Senior Nursing Supervisor or Clinic Nursing Supervisor at the specified Health Center. c. For absence due to illness, notification must be made by 8:30 A.M. of the assignment day. For other absences, 48 hour notice must be provided prior to assignment date. d. Whenever possible, the Fire Rescue Unit will provide a replacement worker from the pool of paramedics who have successfully completed and received a certificate of completion for the "Paramedic Continuing Education Course for Immunizations" from Palm Beach Codamunity College. C. General Provisions 1. The paramedics working in the immunization programs are not employees of the PBCPHU and in no event shall the paramedics represent themselves as agents, officers, servants or employees of the PBCPHU. 2. The Fire Rescue Unit will provide the PBCPHU with the name and telephone number of a designated person to act as a liaison or contact person for the immunization programs. 6. The PBCPHU's Public Health nurse may be considered the lead worker of the immunization programs, however, the Public Health nurse is not a supervisor of the paramedics. 7. This Agreement shah be effective for an indef'mite duration commencing upon the date of signature of both parties. This Agreement may be terminated with or without cause, by either party within sixty(60) days advanced written'notice to the other party delivered by registered or certified mall with the United States Postal Services, return receipt reqUested, by overnight delivery service (for which evidence of the deliver if obtained by the sender), or by hand delivery.
