R95-/~// A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF BOYNTON BEACH, FLORIDA, AUTHORIZING AND DIRECTING THE MAYOR AND CITY CLERK TO EXECUTE A TRAINING CENTER AGREEMENT BETWEEN THE CITY OF BOYNTON BEACH, FLORIDA AND THE AMERICAN HEART ASSOCIATION, PROVIDING FOR INSTRUCTION OF CPR COURSES BY THE CITY'S FIRE-RESCUE DEPARTMENT; AND PROVIDING AN EFFECTIVE DATE. WHEREAS, the City of Boynton Beach, through it's Fire-Rescue Department, has taken an active role in promoting CPR Training; and WHEREAS, the City Commission has deemed it to be in the best interests of the citizens and residents of the City of Boynton Beach to enter into an Agreement with the American Heart Association Emergency Cardiac Care Training Program. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF BOYNTON BEACH, FLORIDA THAT: ~ The City Commission of the City of Boynton Beach, Florida, hereby authorizes the Mayor and City Clerk to execute an Agreement between the City of Boynton Beach and the American Heart Association, a copy of which Agreement is attached hereto as Exhibit "A". ~ This Resolution shall take effect immediately upon passage. The Training Center is solely re.gponai, ble for carrying out its responsibilities under this agreement. Employees of the Training Centers ara not, and shall not be considered, employees of the AltA. A Training Center may not. incur any obligations c~r liabilities for the AHA. RF&KH FTRF-RF~CI!F nFPT hereinafter designated the BLS/PBLS Training Center and the American Heart Association, hereinafter designated as the region, concerning the conduct of a program in Basic Life Support. Under this"agreement, the BLS/PBLS Training Center commencing on JAN, , ~roo will conduct a BLS/PBLS program which shall meet the training standards of the AHA/FA as specified in this manual. The following conditions outline the specific responsibilities of the local component CPR Committee, the Training. Center Authorized Representative, designated Instructor-Trainer, and the Affiliate Faculty Representative in regards to conducting BLS/PBLS training as a Training Center. The American Heart Association Local Responsibilities ~r~: Component, CPR C~ittee 1. Verify that the BLS/PBLS Training program is being conducted in accordance with the American Heart Association program standards. 2. The AHA local component CPR will provide Instructor-Trainer training and retraining programs to accommodate the training needs of the agency. 3. Issue textbooks, roster forms, course completion cards and other related CPR materials to the designated representative of the Training Center. 4. Provide on-going consultation and updates regarding CPR policies and procedures to the Training Center's designated Instructor- Trainer. i. Maintain a pyramid training structure, with an Instructor-Trainer designated to be responsible for adherence to the most current American Heart Association standards and for providing Instructor training adequate ta meet their n~eds. 2. Require that its Instructor-Trainers maintain current American Heart Association training. 3. Agree to have programs and records monitored by the American Heart Associetion at a mutually agreeable time. 4. Formally acknowledge in all courses that any fees for BLS/PBLS training do not represent revenue to the American Heart Association. 5. Provide adequate equipment {adult, child and infant manikins} and space to conduct training programs. 6. Notify the local component within 30 days of the departure of the designated Instructor-Trainer. Responsibilities of the Designated Instructor-Trainer mre: 1. Serves as the contact and resource person between the Training Center and the local component. 2. Serves as resource and contact person for all Instructors assigned to the Training Center. 3. Adheres to standards and guidelines of the American Heart Association, Florida Affiliate for CPR/£CC. 4. Coordinates community courses offered by the Training Center. 5. Offer the following programs as needed: Basic Courses A-F ~6. Recruits and trains Instructors to meet the instructional needs of the Training Center. 7. Provide to'the local committee or component, on June 15 of each year, an updated roster of Instructors. 10. Maintaim names, dates of training/retraining for 3 years for all BLS records. {A longer.period may be needed due to requirements of outside regulatory agencies.} 11. Provide the local component office with a schedule of courses indicating which courses are' offered to the public. It should be understood that all courses are subject to monitoring by the local component. 12. Retraining of all faculty assigned to the center. 13. Participate in the Training Center site visit every second year in conjunction with the local component. 14. Submit the CPR semiannual report as requested by AHA. Responsibilities of the BLS/PBLS Affiliate Faculty Representative 1. Serves as liaisonI between the Training Center component. 2. Monitors compliance with AHA standards/guidelines. 3. Conducts site visits. and the local Termination of A~reement This agreement may be terminated at any time by either party hereto upon 60 days written notice of intent to terminate. Upon closure of a Training Center, all records and monitoring forms must be released to the local American Heart Association component.
