5 A RESOLUTION OF THE CITY COMMISSION OF
6 BOYNTON BEACH, FLORIDA, APPROVING THE ONE
7 YEAR RENEWAL OF THE CONTRACT FOR LIFE AND
8 DISABILITY INSURANCE WITH HARTFORD LIFE
9 AND ACCIDENT INSURANCE COMPANY; AND
to PROVIDING AN EFFECTIVE DATE.

13 WHEREAS, on September 18, 2008, the City Commission of the City of Boynton
14 Beach, via resolution 08-111, approved a Two Year Contract for Life and Disability Insurance
15 Plans with Hartford Life and Accident Insurance Company; and
\6 WHEREAS, the contract approved by the City Commission provided for two (2)
17 additional one (1) year extensions; and
18 WHEREAS, the City Commission of the City of Boynton Beach. upon
\9 recommendation of staff, deems it to be in the best interests of the residents and citizens of the
20 City of Boynton Beach to approve the one year renewal of the Contract for Life and Disability
21 Insurance with Hartford Life and Accident Insurance Company.
22 NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF
23 THE CITY OF BOYNTON BEACH, FLORIDA, THAT:
2 Section I. The foregoing "Whereas" clauses are hereby ratified and confirmed
25 as being true and correct and are hereby made a specific part of this Resolution upon adoption
2 hereof.
2 Section 2. The City Commission of the City of Boynton Beach, Florida does
2 hereby approve the one year renewal of the Contract for Life and Disability Insurance with
