RIO- II" 2 3 A RESOLUTION OF THE CITY COMMISSION OF THE 4 CITY OF BOYNTON BEACH, FLORIDA APPROVING THE 5 MEMORANDUM OF AGREEMENT AND AUTHORIZING 6 THE CITY MANAGER TO EXECUTE A THREE (3) YEAR 7 MEMORANDUM OF AGREEMENT BETWEEN THE CITY 8 OF BOYNTON BEACH AND THE AREA AGENCY ON 9 AGING OF PALM BEACHffREASURE COAST, INC., 10 ALLOWING THE BOYNTON BEACH FIRE RESCUE 11 DEP ARTMENT TO PARTICIPATE IN THE SENIOR 12 IMMUNIZA TION OUTREACH PROJECT; AND PROVIDING 13 FOR AN EFFECTIVE DATE. 14 15 WHEREAS, the goal of the Area Agency on Aging (AAA) Senior Immunization 16 Outreach Project is to reduce mortality and morbidity among the 60 and older population 17 from vaccine preventable diseases; and 18 WHEREAS, this three (3) year Agreement will allow Boynton Beach Fire Rescue 19 Department the ability to provide vaccines to the residents of the City of Boynton Beach at no 20 cost as has been done for the previous seven years; and 21 WHEREAS, the City Commission finds it to be in the best interest of the citizens and 22 residents of the City of Boynton Beach to approve the Memorandum of Agreement and 23 authorize the City Manager to execute the Memorandum of Agreement between the City and 24 the Area Agency on Aging of Palm Beach/Treasure Coast, Inc. 25 NOW THEREFORE BE IT ORDAINED BY THE CITY COMMISSION OF 26 THE CITY OF BOYNTON BEACH, FLORIDA, THAT: 27 Section 1. The foregoing "WHEREAS" clauses are hereby ratified and confirmed 28 as being true and correct and are hereby made a specific part of this Resolution. 29 Section 2. The goal of the Senior Immunization Outreach Project is to reduce mortality and morbidity among the 60 and older population from vaccine preventable diseases by increasing the number of seniors who receive influenza, pneumococcal and tetanus vaccines. This Memorandum of Agreement, dated -n en Je/yy, /ifl .rL ~ 2010, is between the Area Agency on Aging of Palm Beachrrreasure Coast, Inc., dba as Your Aging Resource Center (hereinafter referred to as Your Aging Resource Center) and the City of Boynton Beach (hereinafter referred to as the City). In consideration of the mutual promises and covenants herein contained, the parties agree as follows: Term of Service: This Memorandum of Agreement shall be for the period of September 1, 2010 - August 31/ 2013. 1. The City, through its Fire Rescue Unitt agrees to administer influenza, pneumococcal and tetanus vaccinations to low-income, underserved and/or at-risk older adults, 60 and older and their caregivers who present at host sites. Host sites wlll include senior centers, senior residence communities and congregate meal sites located within the City's Fire Rescue Unit service boundaries. The number of total vaccinations administered wlll be based upon mutual agreement with the Fire Rescue Chief and Your Aging Resource Center, as well as vaccine availability. 2. Your Aging Resource Center will coordinate vaccination clinic dates and times with the City's Fire Rescue Chief at least four to six weeks in advance. 3. Immunizations will be administered to at-risk, underserved seniors and their caregivers each year, starting between mid-October to late October (depending upon manufacturer's shipment date and distribution from Palm Beach County Health Care District). Coordinate the procurement of vaccines and supplies from the Health Care District Pharmacy and deliver to the paramedics at each clinic site on the day of the scheduled clinic. 2. Ensure vaccines are handled and stored at Your Aging Resource Center in accordance with safe practices and procedures prescribed by Palm Beach County Health Department. 3. Target vaccination outreach efforts to low-income, underserved and/or at-risk older adults 60 and older and their caregivers. 4. Secure host sites for Individual clinics In coordination and consultation with the City of Boynton Beach Fire Rescue; promote the vaccination campaign within participating senior communities; and arrange for paramedics to administer vaccinations. 5. Ensure that vaccinees complete all required forms in accordance with HIPPA guidelines and receive current language-appropriate Vaccine Information Statements as prescribed by the Center for Disease Control prior to receiving vaccinations. Termination! Extension: This Memorandum of Agreement is subject to termination, prior to its expiration, upon either party delivering to the other party written notice of intention to terminate this agreement, which will become effective immediately thereafter, or later upon written mutual agreement of the parties. This Memorandum of Agreement may be extended beyond the Initial period upon written mutual agreement of the parties. Governing Law: This Memorandum of Agreement will be interpreted and construed in accordance with and governed and enforced by the laws of the state of Florida.
