5 THE CITY MANAGER AND CITY CLERK TO EXECUTE A SECOND
6 ADDENDUMS TO YOUTH EMPOWERMENT CENTER SERVICES
7 AGREEMENT BETWEEN THE CITY OF BOYNTON BEACH,
8 FLORIDA, AND RM LEE COMMUNITY DEVELOPMENT CENTER,
9 INC., RICHARD BROCHU, TEVIS GARDENHIRE AND KAYLA
10 LUNDY FOR ADDITIONAL UNSPENT GRANT FUNDING FOR
11 SERVICE PROVIDERS WHO WILL OFFER PROGRAMS THROUGH
12 DECEMBER 31,2010; AND PROVIDING AN EFFECTIVE DATE.

14 WHEREAS, the City of Boynton Beach has entered into an agreement with the Board
15 of County Commissioners, Palm Beach County, Florida for the concept of developing a Youth
16 Empowerment Center to curtail violent and criminal behavior by providing holistic activities
17 geared toward improving the livelihood of the participants; and
18 WHEREAS, on September 21, 2010 the City Commission approved the first
19 Amendments to the Interlocal Agreement between the City of Boynton Beach and the Service
20 Proviers to continue implementation of the Youth Violence Prevention Program through
21 December 31, 2010; and
22 WHEREAS, the County has received approval from the Florida Department of Law
23 Enforcement to reallocate existing unspent grant funds to provide additional dollars for the
24 Service Providers who will ofTer programs at the Boynton Beach Youth Empowerment Center
25 through December 31, 2010.
26 NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF
27 THE CITY OF BOYNTON BEACH, FLORIDA, THAT:
28 Section 1. The foregoing "Whereas" clauses are hereby ratified and confirmed as
29 being true and correct and are hereby made a specific part of this Resolution upon adoption
30 hereof.

I Section 2. The City Commission of the City of Boynton Beach, Florida does

2 I hereby approve and authorize the City Manager and City Clerk to execute four (4) seperate
3 second Addendums to Youth Empowerment Center Services Agreement with RM Lee
4 Community Development Center, Inc., Richard Brochu, Facilitator for Hooked on Fishing Not
5 on Drugs, Tevis Gardenhire, Facilitator for Art is Motivation and Kayla Lundy, Facilitator for
6 Success Teens, to provide additional unspent grant funds to the Service Providers who will
7 continue to offer programs at the Boynton Beach Youth Empowerment Center through
8 December 31, 2010, copies of the four (4) separate Second Addendum to Service Agreements
9 are attached hereto and made a part hereof as Composite Exhibit "A"
10 Section 3. This Resolution will become effective immediately upon passage.

SECOND ADDENDUM TO SERVICES AGREEMENT FOR
Youth Empowerment Center
This Second Addendum shall take effect on signature by both parties.
THE CITY OF BOYNTON BEACH, FLORIDA, a municipal corporation,
hereinafter referred to as "CITY",
and
Richard Brochu, Facilitator for Hooked on Fishing Not on Drugs, with an address
of 7067 Peninsular Court, Lake Worth, FL 33467, hereinafter referred to as
"PROVIDER".
WITNESSETH:
I. The Agreement between the CITY and the PROVIDER approved by the City
Commission on June I, 2010, via Resolution R 10-68 is amended as set forth below.
2. All other terms and conditions of the Agreement not specifically amended shall remain in
full force and effect for the balance of the term of the agreement.
In consideration of the mutual terms and conditions, promises, covenants and payments
hereinafter set forth, CITY and DEVELOPER agree as follows:
2.1 Contract Amount. In consideration of the satisfactory performance of the Services
by Provider, and the performance by Provider of all of its other duties and
obligations as set forth in this Agreement, Provider shall be entitled to
reimbursement in an amount not to exceed $15,700 ("Contract Amount") based
on the fees set forth in Exhibit "A" attached hereto. The Contract Amount shall
be the sole amount reimbursed to Provider in connection with the rendition of the
Services and the performance of any and all of its other obligations hereunder and
shall include any out-of-pocket or other expenses, including travel expenses,
incurred by Provider. Payment to the Provider by the City is contingent upon
reimbursement from the County in accordance with the terms of the Interlocal
Agreement

Commission on June 1,2010, via Resolution RI0-70 is amended as set forth below.
2. All other terms and conditions of the Agreement not specifically amended shall remain in

hereinafter set forth, CITY and DEVELOPER agree as follows:

"PROVIDER" .
WITNESSETH:
I. The Agreement between the CITY and the PROVIDER approved by the City
Commission on June 1, 2010, via Resolution R 1 0-69 is amended as set forth below.
2. All other terms and conditions of the Agreement not specifically amended shall remain in
full force and effect for the balance of the term of the agreement.
In consideration of the mutual terms and conditions, promises, covenants and payments
hereinafter set forth, CITY and DEVELOPER agree as follows:
2.1 Contract Amount. In consideration of the satisfactory performance of the Services
by Provider, and the performance by Provider of all of its other duties and
obligations as set forth in this Agreement, Provider shall be entitled to
reimbursement in an amount not to exceed $35,712 ("Contract Amount") based
on the fees set forth in Exhibit "A" attached hereto. The Contract Amount shall
be the sole amount reimbursed to Provider in connection with the rendition of the
Services and the performance of any and all of its other obligations hereunder and
shall include any out-of-pocket or other expenses, including travel expenses,
incurred by Provider. Payment to the Provider by the City is contingent upon
reimbursement from the County in accordance with the terms of the Interlocal
Agreement
