A RESOLUTION OF THE CITY COMMISSION OF
THE CITY OF BOYNTON BEACH, FLORIDA,
APPROVING AN ADDENDUM TO THE ANNUAL
CONTRACT FOR JANITORIAL SERVICE FOR THE
LINKS AT BOYNTON BEACH GOLF COURSE
WITH CLEANING QUALITY SERVICES US CORP.,
IN THE AMOUNT OF $23,472.00; AUTHORIZING
THE CITY MANAGER TO EXECUTE THE
ADDENDUM; AND PROVIDING AN EFFECTIVE
DATE.
WHEREAS, on December 18, 2008, the City Commission approved an Annual
Contract for Janitorial Service for the Links at Boynton Beach Golf Course with Cleaning
Quality Services US Corp; and
WHEREAS, the original bid called for three (3) additional one -year renewal periods
which was inadvertently omitted from the Contract; and
WHEREAS, the first one year renewal was from December 16, 2009 to December 15,
2010; and
WHEREAS, it is recommended to approve the Addendum to the Contract for
Janitorial Services for the Links at Boynton Beach with Cleaning Quality Service US Corp.,
of Hollywood, Florida for the two additional one -year renewal periods.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION
OF THE CITY OF BOYNTON BEACH, FLORIDA, THAT:
Section 1 . The foregoing "Whereas" clauses are hereby ratified and confirmed as
being true and correct and are hereby made a specific part of this Resolution upon adoption
hereof.

Section 2. The City Commission of the City of Boynton Beach, Florida, hereby
approves an Addendum to the Contract for Janitorial Service for the Links at Boynton Beach
Golf Course with Cleaning Quality Service US Corp., of Hollywood, Florida with an annual
expenditure of $23,472.00 and authorizes the City Manager to execute the Addendum
between the parties, a copy of which is attached hereto as Exhibit "A ".
Section 3. That this Resolution shall become effective immediately.

ADDENDUM TO CONTRACT FOR
JANITORIAL SERVICES FOR THE LINKS AT BOYNTON BEACH
This Addendum shall take effect on signature by both parties.
THE CITY OF BOYNTON BEACH, FLORIDA, a municipal corporation,
hereinafter referred to as "CITY ",
and
CLEANING QUALITY SERVICE U.S. CORP, a Florida corporation, hereinafter
referred to as "VENDOR"
WITNESSETH:
The Agreement between the CITY and the VENDOR entered into the 1 st day of January,
2009 is amended as follows:
In consideration of the mutual terms and conditions, promises, covenants and
payments hereinafter set forth, CITY and CLEANING QUALITY SERVICES US
CORP agree as follows:
Section 3. TIME FOR PERFORMANCE. Work under this contract shall commence
upon the giving of written notice by the City to the Bidder to proceed.
Bidder shall perform all services and provide all work required pursuant to
this agreement by December 16, 2011 unless an extension of such time is
granted in writing by the City. There are two (2) additional one (1) year
renewal periods available at the sole option of the City.
2. All other terms and conditions of the Agreement not specifically amended shall remain in
full force and effect for the balance of the term of the agreement.
