3 A RESOLUTION OF THE CITY COMMISSION OF
4 THE CITY OF BOYNTON BEACH, FLORIDA,
5 APPROVING AND AUTHORIZING THE CITY
6 MANAGER AND CITY CLERK TO EXECUTE AN
7 ADDENDUM TO YOUTH EMPOWERMENT
8 CENTER SERVICES AGREEMENT BETWEEN THE
9 CITY OF BOYNTON BEACH, FLORIDA, AND RM
10 LEE COMMUNITY DEVELOPMENT CENTER, INC.,
11 INC., TO EXTEND THE TERM TO MARCH 31, 2011;
12 AND PROVIDING AN EFFECTIVE DATE.

14 WHEREAS, the City of Boynton Beach has entered into an agreement with the Board
15 of County Commissioners, Palm Beach County, Florida for the concept of developing a
16 Youth Empowerment Center to curtail violent and criminal behavior by providing holistic
17 activities geared toward improving the livelihood of the participants; and
18 WHEREAS, on June 1, 2010, the City of Boynton Beach approved a Services
19 Agreement for Youth Empowerment Center with RM Lee Community Development Center,
20 Inc; and
2Ji WHEREAS, funds are still available to continue to offer the program ($6,000)
2:[ , therefore the term of the Agreement is being extended to March 31, 2011.

2:1 NOW, THEREFORE, BE IT RESOLVED BY THE CITY
24 COMMISSION OF THE CITY OF BOYNTON BEACH, FLORIDA, THAT:

2:1 Section 1. The foregoing "Whereas" clauses are hereby ratified and confirmed as

24 being true and correct and are hereby made a specific part of this Resolution upon adoption

ADDENDUM TO SERVICES AGREEMENT FOR
Youth Empowerment Center
This Addendum shall take effect on signature by both parties.
THE CITY OF BOYNTON BEACH, FLORIDA, a municipal corporation,
hereinafter referred to as "CITY",
and
RM Lee, Community Development Center, 900 N. Seacrest Blvd., Boynton Beach,
Florida, 33435, hereinafter referred to as "PROVIDER".
WITNESSETH:
l. The Agreement between the CITY and the PROVIDER approved by the City
Commission on June 1,2010, via Resolution RIO-69 is amended as set forth below.
2. All other terms and conditions of the Agreement not specifically amended shall remain in
full force and effect for the balance of the term of the agreement.
In consideration of the mutual terms and conditions, promises, covenants and payments
hereinafter set forth, CITY and DEVELOPER agree as follows:
2.1 Contract Amount. In consideration of the satisfactory performance of the
Services by Provider, and the performance by Provider of all of its other duties
and obligations as set forth in this Agreement, Provider shall be entitled to
reimbursement in an amount not to exceed $41,712 ("Contract Amount") based
on the fees set forth in Exhibit "A" attached hereto. The Contract Amount shall
be the sole amount reimbursed to Provider in connection with the rendition of the
Services and the performance of any and all of its other obligations hereunder and
shall include any out-of-pocket or other expenses, including travel expenses,
incurred by Provider. Payment to the Provider by the City is contingent upon
reimbursement from the County in accordance with the terms of the Interlocal
Agreement
3. The PROVIDER does accept the term of the engagement shall continue until March 31, 20 II.
