07.04.01 - Work Order Requests CITY OF BOYNTON BEACH, FLORIDA ADMINISTRATIVE POLICY MANUAL CHAPTER: 07 City Facilities POLICY NO.: 07.04.01 SECTION: 04 Facilities Management PAGE: 1 of 2 SUJBECT: 01 Work Order Requests PURPOSE AND SCOPE The purpose of this directive is to establish procedures for requesting services by Facilities Management and to outline the process of a Work Order Request. The scope shall encompass all City Departments that require and/or would request service from this Division. PROCEDURES 1) Work Order Request Submittal: A) Non-Emergency -All Work Order Requests should be entered into the H.T.E. System. B) Emergency Work Order Requests during Normal Working Hours – All Work Order Request should be entered into the H.T.E. System after notifying Facilities Management or Public Works Main Answering Point via phone or radio. Emergency Phone #s during work hours: Ext. 6200 – M.A.P. Ext. 6020 – Facilities Supv. Ext. 6214 – Facilities Office Ext. 6580 – Facilities Office C) After Hours Emergencies should be reported to Communications at 742-6035/6036 for notification of the appropriate personnel. D) Note: Emergencies are conditions that present a general Life/Health/Safety Hazard. 2) Work Order Request Computer Entry: A) Enter the H.T.E. Program Step 1: #1 H.T.E. Inc Application Menu Step 2: #15 Work Orders /Facility Mgmt Step 3: #1 Request Processing Menu Step 4: #1 Work Request Maintenance Step 5: Add /F6 1 2 Step 6: Enter Requestor Name, Requesting Department, Request Origin, Request Category (must contain the prefix FGMT in order to be forwarded to Facilities), Work Type (SC/Scheduled for Non-Emergencies & US/Unscheduled for Emergencies), Facility ID, Requested Completion Date, and Short Description /Long Description. OK/Enter to go to the second screen and then OK/Enter to submit; you’ll be returned to the original entry screen. 3) Departmental Responsibilities: A) Requestor should be the person most familiar with the request and its details. B) Account Number with a Project Number, if appropriate, or a Police Department Case Report # for an accident related repair should be included in the Long Description of the Work Order Request for any work other than normal maintenance or basic repairs.
