12.01.09 - Incident Review Board CITY OF BOYNTON BEACH, FLORIDA ADMINISTRATIVE POLICY MANUAL CHAPTER: 12 Risk Management Policy No. 12.01.09 SECTION: 01 Risk Management Policies Page: 1 of 2 SUBJECT: 09 Incident Review Board (formerly Accident Review Board) PURPOSE: To establish an Incident Review Board to review all incidents and injuries to determine preventability or non-preventability on the part of the employee. POLICY: 1. Each work related injury and/or motor vehicle accident shall be investigated by the supervisor of the employee involved as soon as possible after the incident has occurred, following the investigating and reporting procedures outlined in APM Policy 12.01.02. 2. The affected department will complete the RM-102, Supervisor’s Report of Incident (revised 07/2009) and forward to Risk Management within 24 hours of the incident. 3. The Risk Management Department will review the form for completeness and compile the required information for the Incident Review Board (IRB). 4. The IRB will meet the 2nd and 4th Tuesday of each month to review all incidents, which have occurred within the time frame, and to hear any appeals of determination. 5. The IRB will review the RM-102 forms completed by departments, with supporting documentation, and determine preventability or non-preventability on the part of the employee. 6. The IRB shall notify the employee, through the Department Head, of this determination in writing and of the right to appear before the IRB to appeal the IRB’s decision. Appeal by the employee must be requested within 30 days of the IRB’s decision. 1 2 COMPOSITION: 1. The IRB shall consist of seven (7) members to include Safety Coordinators, Alternates, or other designees, appointed by the Department Head, from the following Departments/Divisions : Fire/EMS Risk Management Police Utilities Public Works Blue Collar Union Recreation & Parks 2. The Chairperson of the IRB will be the primary member from the Risk Management Department. The Vice Chairperson will be appointed by the City Manager. AUTHORITY: 1. The IRB is a Standing Board of the City of Boynton Beach Safety Committee and derives its authority from the Committee. 2. The IRB shall review incidents from all departments/divisions.
