A RESOLUTION OF THE CITY COMMISSION OF
4 THE CITY OF BOYNTON BEACH, FLORIDA,
APPROVING A FIRST ADDENDUM TO THE
"UTILITIES DEPARTMENT TWO YEAR
JANITORIAL SERVICES CONTRACT" WITH
UNITED STATES SERVICE INDUSTRIES, INC., OF
41 FORT MYERS IN THE AMOUNT OF $26,768.88
14 ANNUALLY; AUTHORIZING THE CITY MANAGER
1 TO SIGN THE ADDENDUM; AND PROVIDING AN
14 EFFECTIVE DATE.

10 WHEREAS, on June 21, 2011, the City Commission approved a two (2) year contract
16 between the City of Boynton Beach and USSI to provide janitorial services for the Utilities
17 Administration Building, the East Water Treatment Plant and the West Water Treatment
16 i Plant; and
1 WHEREAS, the contract contains an option to extend for an additional one year
26 period and the Vendor is requesting an increase due to the rising costs of products and labor;
21 and
2. WHEREAS, the Utilities Department has reviewed the request and is recommending I
2 a 2% increase for the renewal term to which USSI is agreeable; and
2 WHEREAS, the City Commission of the City of Boynton Beach upon
2 - recommendation of staff, deems it to be in the best interest of the citizens of the City of
2: Boynton Beach to approve the First Amendment to the "Utilities Department Two Year
2 Janitorial Services Contract" with USSI in the amount of $26,768.88, annually; and
2: authorizes the City Manager to sign the First Addendum.
2• NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION

1 Section 1. The foregoing "Whereas" clauses are hereby ratified and confirmed as
2: ' being true and correct and are hereby made a specific part of this Resolution upon adoption
31 hereof.
41 Section 2. The City Commission of the City of Boynton Beach, Florida, hereby
6 approves a First Addendum to the "Utilities Department Two Year Janitorial Services
61 Contract" with United States Service Industries, Inc., of Fort Myers, Florida with an annual
/ expenditure of $26,768.88 and authorizes the City Manager to sign the Addendum between
6 the parties, a copy of which is attached hereto as Exhibit "A ".
Section 3. That this Resolution shall become effective immediately.

FIRST ADDENDUM TO THE "UTILITIES DEPARTMENT TWO YEAR
JANITORIAL SERVICES CONTRACT"
This First Addendum shall take effect on signature by both parties.
THE CITY OF BOYNTON BEACH, FLORIDA, a municipal corporation, hereinafter referred to as
"CITY ",
and
UNITED STATES SERVICE INDUSTRIES (USSI) of Fort Myers, Florida hereinafter referred to as
"CONTRACTOR"
WITNESSETH
1 The Contract between the CITY and CONTRACTOR entered into the 11 of July, 2011 is
amended as follows -
In consideration of the mutual terms and conditions, promises, covenants and payments
hereinafter set forth, CITY and CONTRACTOR agree as follows:
Section 3. TIME FOR PERFORMANCE. Work under this contract shall commence
August 1, 2014 through July 31, 2015.
Section 4 a. Payment for the work provided by Contractor shall be made as provided on
Exhibit "A" attached hereto, provided that the total amount of payment to the
Contractor shall not exceed $2,230.74 per month, $26,768.88 per year without
Express written modification of the Contract signed by the City.
2. All other terms and conditions of the Contract not specifically amended shall remain in full force
and effect for the balance of the term of the Contract

Dear Ms. Casavant:
The current contract renewal period for the "Two Year Janitorial Services Contract for the Utilities
Department" will expire July 31, 2014. The City of Boynton Beach Utilities Department is pleased with
your service and the City would like to renew the existing contract under the same terms and conditions.
In consideration of USSI's increased costs for supplies, the City of Boynton Beach is agreeable to a 2%
increase in the contracted annual fee subject to City Commission approval. If approved, the annual
payment to USSI effective August 1, 2014 would be $26,768.88 ($2,230.74 /month).
