R01-310 RESOLUTION R 01- ~ im A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF BOYNTON BEACH, FLOPdI)A, AUTHORIZING TId~ CITY MANAGER TO EXECUTE AN ADDENDUM TO THE EMERGENCY MEDICAL SERVICE DIRECTOR CONTRACT BETWEEN THE CITY OF BOYNTON BEACH AND KENNETH A. SCHEPPKE, M.D.; AND PROVIDING AN EFFECTIVE DATE. WIIEREAS, on December 5, 2000, the City entered into an Agreement with Scheppke, for a one year period to oversee the Emergency Medical Services as ' Fire Rescue/EMTS, Paramedics and Lifeguards; and WIIEREAS, the Agreement contained an option to renew for an additional year period, which option is being exercised by the City and Dr. Scheppke, for an annual salary mcrease of $2,500; NOW, THEREFORE, BE IT RESOLVED BY TIlE CITY COMMISSION OF TIlE CITY OF BOYNTON BEACH, FLORIDA, THAT: Section 1. The City Commission of the City of Boynton Beach, Florida authorize the City Manager to execute an Addendum to the Emergency Service Director Contract between the City of Boynton Beach and Kenneth a copy of said Addendum being attached hereto as Exhibit "A". Section2. This Resolution will become effective immediately upon PASSED AND ADOPTED this ~ day of December, 2001. kTTEST: CITY OF J~OYNTON BEAJ~H, FLORIDA Mafor / mmission~r Commissioner 'Clerk FIRST ADDENDUM TO EMERGENCY MEDICAL SERVICE DIRECTORCONTRACT This Addendum, dated the day of December, 2001, amends the original Contract between the City 6f Boynton Beach and KENNETH A. SCHEPPKE, M.D.. Unless expressly modified by this Addendum, the original terms of the Appointment Contract shall remain in full force. This Contract Appointment is amended as follows: Recitals, 11. (Amended) The City will pay the Medical Director the sum of Twenty Tl~ee Thousand Five Hundred ($23,500) Dollars per year, with the monthly rate being Nineteen Hundred Fifty Eight Dollars and 33/100 ($1,95&33) per month for twelve (12) months, payable at the end of each month for services outlined in this contract. Article 20. (Amended) The term of this agreement shall be for twelve months effective January 1, 2002 through December 31, 2002. The Deputy Director of Financial Services may renew the contract, at the same terms, and conditions, for one (1) additional one (1) year term, subject to vendor acceptance, satisfactory performance and determination that renewal will be in the best interest of the City. In the event of a conflict between the original Contract and this Addendum, the Addendum shall control. The foregoing provisions represent the agreement of the parties and become effect on January 1, 2002, subject to ratification by the City Commission.
